Family Medicine, Geriatrics and Wellness in Lower Gwynedd, Pennsylvania was recently recognized as a NCQA Level III Patient-Centered Medical Home. Practice leader, Joseph Mambu, MD answered a few questions about his practice's experience and offered some persuasive numbers: "As a participant in the SEPA Chronic Care Collaborative, we were financially incented to obtain our NCQA medical home certification...Our PPC-PCMH™ earned us approximately $57,000 for year one and $106,000 in both years two and three of the collaborative."

TransforMED is focused on practice redesign and affiliated with the American
Academy of Family Physicians (AAFP). TransforMED is studying and implementing
transformed models of high performance practices that meet the needs of
both patients and practices. 
TransforMED
also coordinates a residency demonstration initiative known as P4
– P to the fourth power – which stands for Preparing the Personal
Physician for Practice. P4 is leading the transformation of
family medicine residency education training to prepare tomorrow's physicians
for new models of care and demonstrate the value of science-based, high
quality, patient centered primary care. 
27 AAFP Chapters are now taking advantage of TransforMED's new AAFP Chapter Partner program. The partnership provides TransforMED tools, access to TransforMED speakers with expertise in areas related to the patient-centered medical home, special events, and other TransforMED resources to AAFP Chapters who join the program. 
Three members of the Preparing the Personal Physician for Practice Executive committee received the Society of Teachers of Family Medicine (STFM) Advocate Award. The Advocate Award is designed to recognize excellence in the field of political advocacy at the local, state and national level. 
TransforMED and CareFirst Blue Cross Blue Shield conducted their second patient-centered medical home (PCMH) learning collaborative at the Baltimore Marriott Inner Harbor in Baltimore, Maryland on August 8, 2009. The learning collaborative celebrated the pilot practices' PCMH transformational efforts thus far and focused on Care Coordination and Care Management.
